The forms below are required for each camper. Forms can be downloaded, completed, and then uploaded into your camper's registration account. Health History Form

Health history forms should be completed for each camper for the 2026 season.

Physical Exam Form

Physical Exams should be submitted for each camper for the 2026 season. There are a few ways you can do this:

Download our form here to be filled out by a registered physician or nurse practitioner needs to fill out this form.
Submit another health exam/physical that has been done within 24 months of camp attendance dates. This can be a physical form for another program, school, sports, or even a detailed visit summary from your camper’s last doctors visit approving them for physical activities as long as it has been completed by a licensed physician or nurse practitioner.
Located your physical exam form from last summer. These will be accepted if they are within 24 months of the camp program and should be re-uploaded on the registration site account.

We recognize that getting in for an annual physical may be a challenge, because of this if you are not able to get one of the above physical exam forms completed by June 1, please let us know.
